Medical Release Form

Sometimes our dancers are not feeling their best while at dance class. We have medication available to assist your dancers if needed, but will need prior parent approval before administering any. If you would like us to offer medication if needed, please fill out the following form and return to our office staff. 
We keep the following things on hand: 

• Ibuprofen & Children's Ibuprofen
• Tylenol & Children's Tylenol
• Tums & Children's stomach aid

Please list any of the above medications you would be okay with us administering to your dancers as needed. If you do not have a signed release form on file, we will not allow any medication to be given to your dancer. ​
